Signing-up and registering our programs is easy. You can either download the forms and forward them to us by Fax, Email, Mail etc. or you can utilize our on-line registration system for most of the process.
In order to sign-up we need the following information submitted to us at least one month prior to the session start date or desired shipment date. We can arrange for express service but you will incur a $25.00 express service fee plus increased delivery charges (please see the FAQs for more information on Express Service and Late Registration).
The first step requires registering your Parent or Guardian information. This can be done either online or by downloading the form. On-line registration and sign-up saves us time and gets the process underway quicker.

Download Information Packet. Please download and print the information packet which includes, medication information detail, credit card authorization, Physician instruction forms.
Please fax or present a copy of the Physician instruction form directly to the Doctor so that the prescription can be written correctly AND according to the procedures required by New Jersey Pharmacy Laws. Sometimes Doctors don’t like to follow directions but medication deliveries will be delayed without proper information from the prescribing physician(s).
